Cancellation Policy
Your appointment is very important to us. When appointments are made, time is reserved specifically for you. We take extra steps to remind you of your appointment. If you are unable to keep your scheduled time, please contact our office at least 24 hours prior to the appointment. We understand unforeseen circumstances may occur, our office may review these situations at our discretion. Failure to comply with this policy will result in the following charges:
A $50 non-refundable fee will be charged if:
- You do not provide at least 24 hours notice if you need to cancel or reschedule your appointment
- You do not show up to your appointment
In addition to the non-refundable $50 fee, your co-insurance will be collected prior to rescheduling a surgery appointment
- Failure to keep your scheduled appointments may result in termination from our practice
Cancellations MUST be made during normal business hours:
- Monday – Thursday 8-4:30
- Friday 8-3
