Financial Information
Payment: Patients or their legal guardian are responsible for all charges incurred during treatment and must pay for services at the time they are rendered. Ninety days after the date of service there will be a service charge of 1.5% per month (18% per year). The minimum monthly service charge is $2.00. We accept cash, checks, and all major credit cards. A $30 service fee will be applied for returned checks. Outside financing is available through Care Credit-a healthcare financing program. You can apply by scanning the QR code to the right or at: www.carecredit.com/apply. Interest-free payment plans are available for up to 12 months, depending on the amount being financed.
Patients with In-Network Insurance:
Our office participates with Delta Dental, Blue Dental, and Blue Cross Blue Shield. We request payment on the day of surgery based on information we are able to verify from your insurance company prior to surgery. Our business office staff can only provide a courtesy estimate. We will send documentation needed to process your claim. We will assist you with your claim, but it is your responsibility to pursue unpaid claims promptly. If the insurance does not process the claim within 90 days, you will be responsible for this amount. I hereby authorize the payment of insurance benefits directly to the office of Oral Surgery and Dental Implant Center of Iowa City.
Patients with Out-of-Network Insurance:
As a valued service to you, we will submit claims on your behalf to your insurance company. Your insurance policy is a contract between you and your insurance company. The doctors are not part of the contract. Therefore, all charges incurred are your responsibility and you are responsible for your charges whether or not your insurance company pays. If your insurance company has not paid within 90 days of your claim, you will need to contact your insurance company to investigate your claim.
Any reimbursement from your insurance company will be sent to the primary policy holder. In the event that your insurance company sends payment directly to our office, you will be reimbursed.
Divorce situations: The parent who brings the minor child to the initial appointment is responsible for all charges incurred during treatment, regardless of who provides insurance coverage. Our office will not become involved in payment disputes between separated individuals.
